What ages are seen?

All ages are seen.

Are walk-in services available?

Yes, we offer walk-in for Diagnostics Exams. All other exams require an appointment. 

What items should I bring to my visit?

Please bring your ID card, 3rd Party Insurance Card and outside provider prescription. 

Can I receive a copy of my x-ray?

All requests for film will be provided on a CD. 

Can someone else pick up my films for me?

A signed letter of authorization memo and copy of the patients ID card or drivers’ license must be presented. 

How long is my referral good for?

Due to possible changes your provider may make in management of your care, and inherent risk of unnecessary exposure to ionizing radiation, any referrals, or prescriptions, over 30 days old must be renewed by your Primary Care Physician. Exception: Mammography Request 

Can I bring my child/children to my appointment?

All children under the age of eight must have supervision in direct sight of a reliable person at least 13 years of age at all times. Volunteers may serve as supervisors with parental approval. 

Staff cannot assume responsibility for non-medical supervision of children except in a medical emergency.
