1) Log into www.mods.army.mil from a CAC-enabled computer. 2) Select MHA (PHA/DHA/Referral Tracking) and Login. 3) Enter DOD ID number. Click on on PHA tab and select "Start New Survey" to complete questionnaire for PHA Part 1.
When questionnaire for PHA Part 1 is complete, call the DOD VIPRR Virtual Medical Center - 1-844-VMEDCEN (1-844-863-3236) to schedule your PHA Part 2 telephone appointment. Questions? Schedule an appointment for Pre/Post-Deployment health assessments, Automated Neuropsychological Assessment Metric (ANAM) TBI testing and travel medicine by sending a secure message via MHS GENESIS or emailing us. Taking an ANAM assessment is a requirement for all service members within 12 months prior to deploying. Arrange an appointment with an NCR ANAM proctor for individual testing or the proctor can come to your location to test personnel for larger groups. Travel Medicine for Active Duty on personal travel or Civilian personnel call Occupational Health at 301-677-8402 for appointments. Note: Personnel with known hearing loss, previous exam failures, hearing aids, and/or hearing profiles should contact the clinic directly at 301-677-8813 to be seen by an Audiologist.
Soldiers must ensure medical readiness prior to a deployment. Our Medical Readiness Clinic typically processes 5-10 deploying Soldiers per week. If there is a large group (greater than 10 Solders), please have your Personnel Office (S-1) or Unit leaders contact us with a list so we can prepare for an SRP and adequate support for Unit deployment processing.
