Medical Readiness for Permanent Change of Station
Permanent Change of Station in
Continental United States (CONUS) and outside CONUS screening forms are completed through the Medical Readiness Clinic, not your primary care provider. Please do not book an appointment through the appointment line or the patient portal.
Forms can be dropped off at the Primary Care font desk. Please allow 5 duty days for review and we will reach out to schedule an appointment.
Medical Readiness for Deployment
Soldiers must ensure medical readiness prior to a deployment. If there is a large group (greater than 10 Solders), please have your Personnel Office (S-1) or Unit leaders contact us with a list so we can have adequate support for Unit deployment processing.
