Physical Exams

Periodic Health Assessment

The Periodic Health Assessment (PHA) consists of three parts.

The first part is the online questionnaire via AKO. 

     *NOTE: Must state "Members Portion Complete"

 

The second part is done by calling (410) 278-5475 or on a walk in basis to the 3rd floor Occupational Health Clinic for your individual screening by Medical Readiness personnel.

  •      Vision – Annual requirement for all active duty service members
  •      Hearing – This is an annual requirement for all active duty service members.
  •      Based on a determination by Medical Readiness personnel may require an EKG, HIV and additional labs.

The third part is a face-to-face encounter with a healthcare provider to complete the PHA.
 

School Physicals

Airborne, Ranger, Special Forces, SERE, Commission, Warrant Officer.

To begin the process of getting a physical for any of the items listed above, Service members just need to sign in at the front desk of PCMH to be seen by Soldier Physical Exam Personnel to be screened and have any lab work put in and any EKG’s administered prior to making an apt to see the provider.

The paperwork needed for the above physicals is listed below. Once it is filled out please turn it into a Soldier Physical Exam Personnel prior to scheduling your appointment.

Separation Physicals

To begin the process for a separation physical, Service members can sign in at the front desk of the Primary Care to see Soldier Physical Exam personnel on a walk-in basis for screening or any lab.radiology orders.

For Service members that are being involuntarily chaptered are required to have an NCO escort throughout the process and for the final appointment. They must also provide a memo from their commander stating what chapter they are being processed under. The memo can be brought in during the walk-in portion.

Retirement Physicals

Service members can begin the Retirement physical 6 months prior to their planned transition/retirement date.

To begin the process of getting a retirement physical, Service members will need to sign in at the front desk to see Soldier Physical Exam personnel and be screened and have any lab work put in and any EKGs administered.

The paperwork needed for retirement physicals is listed below:

Complete these forms and turn them into a Soldier Physical Exam Personnel.

Reserve and National Guard soldiers will need to bring their ID card and a set of orders activating them to receive care, or a memo from their commander authorizing care on that specific day. For more information please contact Warrior Readiness staff: 410-278-1960

Allow for a minimum of 5-10 business days for all physicals to be signed by a medical Doctor. Once Warrior Readiness has received the complete and signed physicals the staff will contact the Service Member to come pick up the completed physical

Sick Call

Personnel are seen by appointment only. Kirk U.S. Army Health Clinic does offer early appointments for Active Duty members, starting at 6:30 a.m. on normal business days.

Appointments with your Primary Care Manager, or a Specialty appointment, can be made by calling 410-278-5475 / 1-866-756-5475. You may also speak with the TRICARE Nurse Advice line by calling 1-800-874-2273, Select Option 1

Kirk is no longer a full fledged hospital, clinic services are Monday through Friday from 7:30 am to 4:30pm.

 

Emergency Medical Care

Emergency conditions are those that threaten your life, limb or eyesight. TRICARE defines an emergency as a medical, maternity or psychiatric condition that would lead a “prudent layperson” (someone with average knowledge of health and medicine) to believe that a serious medical condition existed, or the absence of medical attention would result in a threat to his or her life, limb or sight and requires immediate medical treatment or which has painful symptoms requiring immediate attention to relieve suffering.

Kirk U.S. Army Health Clinic does not have an emergency room.

  • Emergency care does not require prior authorization
  • Call 911 or go to the nearest emergency room

 

Behavioral Health Emergencies

After normal duty hours, weekends and holidays go immediately to the nearest Emergency Room or call 911 (Kirk U.S. Army Health Clinic does NOT have an Emergency Room).

If you or someone you know is feeling suicidal, during duty hours (7:30 a.m. to 4 p.m.), please contact Behavioral Health immediately or call The National Suicide Prevention Hotline at 1-800-273-8255.

 

Additional Information

Beneficiaries enrolled in TRICARE Prime and assigned to Kirk U.S. Army Health Clinic should contact the appointment call center at (410) 278- 5475 or toll free (866) 756- 5475 to schedule a follow up visit with their assigned Primary Care Manager within 24 hours or the next business day to coordinate follow up care.

Beneficiaries using TRICARE Standard and TRICARE for Life do not require prior authorization during an emergency, applicable annual deductibles and cost shares apply.

Beneficiaries enrolled in TRICARE Prime through the U.S. Family Health Plan must contact their Primary Care Provider or the U.S. Family Health Plan for information on how to obtain follow up care after an emergency.

Aberdeen Proving Ground is comprised of two areas: the Aberdeen Area/APG North and Edgewood Area/APG South. These areas are not connected but are within a short driving distance of one another.

Kirk has an occupational health clinic and limited primary care services for Active Duty members assigned to the 20th Support Command located in the Edgewood Area/APG South.
