Patient-Centered Medical Home

We want you to feel at home with all of your medical needs at Kirk US Army Health Clinic. The evidence- based, patient-centered medical home model of health care relies on a strong relationship between you and your Primary Care Manager (PCM), a trained health professional who will get to know you well and who can help you manage all of your primary care needs. Through a strong relationship with your provider, we can emphasize overall wellness, through preventive medicine, routine screening, chronic disease management, as well as acute care. Your PCM is a member of a team of providers who will be available to see you, manage your health, ensure you are referred to appropriate specialists when required, and who will follow up with you for all of your medical needs.
